How to Access Escalating Nightmares

Escalation Sigils drop from Horadric Caches, which are awarded after completing Strongrooms on Torment I difficulty or higher.
Using the sigil will open the first dungeon in the Escalating Nightmare sequence.
Upon completing each dungeon, a Horadric Portal opens, transporting you directly into the next tier — and revealing the new affixes now in play.
