A reader sent me this story from a local Fox news station in Seattle. Sergeant Sean Collins (2nd BCT, 101st Airborne) was killed in action in Afghanistan on December 12th. Senator Maria Cantwell sent a condolence letter and, in the last paragraph, mistakenly identified Sean as "Bryn". Senator Cantwell from Washington made a mistake, owned it and apologized. The family accepted her apology. That's very good - or at least, the best of a bad situation made worse by a mistake.
